CLBC Eligibility Assessments In Surrey, BC

Community Living British Columbia Eligibility Assessments in BC (to support your CLBC application) Ages 16.5+

Community Living BC (CLBC) is a provincial crown corporation that provides services and supports to adults with developmental disabilities such as intellectual developmental disorder, autism spectrum disorder (ASD), or fetal alcohol spectrum disorder (FASD). To determine eligibility, CLBC requires a psychological assessment that evaluates intellectual functioning, adaptive behaviour, and other relevant factors.

A CLBC assessment provides the documentation and evidence needed to confirm eligibility, helping you access housing supports, community inclusion services, employment supports, and more.

Assessment Intake Form

For ASD, Brief ADHD, Psychoeducational (ADHD, LD, ID) and Giftedness Assessments:

Before your first assessment appointment, we ask that you gather as many relevant supporting documents related to your/ your child's psychological concern as possible, and bring them to your first appointment. This includes any previous psychological reports, reports from SLPs, OTs or PTs, relevant medical records (which you are able to request from your doctor), and school report cards.

As neurodevelopmental disorders (such as ASD, ADHD, LD, and ID) are lifelong, they require a historical background confirmation before a diagnosis can be delivered. Therefore, we ask that all assessment clients bring as many historical documents as possible to your first appointment (even for adult assessments). If you have any concerns about being able to provide historical documents, we can discuss this further in your intake appointment.
